transparency-files.html), the health insurance carrier and/or health maintenance organizations that La
Crescenta Presbyterian Church has contracted with to provide its employee health benefit plans in
response to the federal Transparency in Coverage Rule and includes negotiated service rates and out-of-
network allowed amounts between health plans and healthcare providers.  The machine-readable files
